ORDER AND now, you
are going to prepare
your tender booklet. The booklet shall consist of the
following informations and documents with the paper
back cover marked 

“TENDER FOR THE SUPPLY OF ...  TO THE
FEDERAL MINISTRY OF HEALTH IN NIGERIA?BR>
The booklet will consist of the following:
1, Letter of introduction of your co***ny to the
Honourable minister of HEALTH

2, Your proforma Invoice showing the unit cost (25%
uplift included), Insurance cost and the freight cost
by SEA to Lagos SEAport COVERING ... WORTH OF US$5.5 MILLION.

3, Certificate of registration or incorporation of
your co***ny name in CHINA.

4, Letter of identification of your co***ny name and
address by your bankers in CHINA.

5,Tax clearance receipts of your co***ny if any.
 

This documents must be laminated and made into a
booklet and sent to me via DHL for me to personally
submit it to the appropriate office of the Ministry
for their study and possible acceptance. In the case
of items 1 and 2, it must be written in your
letter-headed papers duly signed and stamped with your
co***ny’s seal for authentifications. The Proforma
Invoice must have the address of the Federal Ministry
of HEALTH as the principal buyer. The letter of
introduction can be written in the below manner:

intention to award a contract for the supply of
US$5.5 MILLION WORTH OF .... We take

provided we receive up front payment by T/T. we are
confident that we undertake the contract for the
supply of your requirements. We enclose herewith our
Proforma Invoice and vital informations and documents
of our co***ny for your study. We hope you will find
favour in our offer and award the contract.
We are looking forward to hear good news from your
Ministry.
