vvNotify Party Name & Address
vvPort of Discharge & Destination
vvOcean Freight Payable – At origin (PREPAID) or at Destination (COLLECT)

由船公司/貨代公司提供“訂倉單”表格
由出口商 (發貨人) 向船公司/貨代公司申請倉位/櫃位
– 出口商 (放貨人) 必須提供一下資料:
v 發貨人名稱,地址,電話號&聯係人
收貨人名稱/地址
被通知人名稱&地址
卸貨港/***終目的地.(***/DEST
海運費繳付方式-原產地（預付）目的地（到付）

v 散貨一件數/尺碼/重量
貨櫃-多少個貨櫃。貨櫃尺碼（20’40’）

Forwarder/Shipping Co. must confirm the below information :
–                        –Booking No, Shipping Order No, ***k Receipt No.
–Vessel name, Voyage No–Closing date& time–Place of Goods Delivery
–S.I. Closing date & time
–The information in the shipping order will be used to issue Bill of Lading after shipment on board.
(*Remarks : For Full Container, Forwarder/Shipping Co. provides 
Exporter/Shipper the FCL Equipment Release/Collection Order to pick 
up the empty Container at the container yard and return after loading.)
貨代/船公司必須確認下列資料
–訂倉號
-船名/航次
截關日期/時間
–卸貨港/目的地（POD/DEST）
–截提單補料日期/時間
貨物出運後訂倉單内的資料將變成正式提單的内容。
備註:有關整櫃貨,貨代公司/船公司會提供“提吉櫃/還重櫃給出口商/發貨人
,以便客戶於貨櫃堆場提吉及將重櫃交還碼頭出運

(2) Mate Receipt
•Ø ONE of the page of shipping order mostly in yellow
Functions :
– Warehouse people will return this mate receipt to the 
  transporter after received the cargo.
– Information on the mate receipt :
––Chop & Sign by warehouse.
–How many packages were received in numbers and full writing?
–To write down in case the cargo is not in good condition, 
 Such as wet, crush, broken……etc.
e)  Cargo receiving date.
(*remarks : If goods are not in good condition, warehouse’s people

(3) Bill of Lading (B/L)
•Functions :
–A Contract
–To exchange the B/L must be against the mate receipt.
–To indicate the option of the goods.
–To prove goods handed over to Forwarder/Shipping Company.
–Must be issued after the shipment on board.
–The  information on the B/L is according to the shipper order and some additional information

–Issued by company owned the vessel
–•House Bill of Lading (HB/L)
–Issued by NVOCC (Non-vessel operating common carrier)

–Alike HB/L but can be issued before shipment on board with against
 goods received by warehouse. “This cargo receipt is not a document of title”•Through Bill of Lading (TB/L)
–A B/L for transshipment to cover origin port, transshipment port and 
 destination port with different countries.
–The Journey of the goods to be transported with excess one vessel
 (Example: 1st leg from USA to HK, 2nd leg from HK to China)

–According to through B/L, which need to change the memo B/L with the 2nd leg
